The following information is required if you are unable to return equipment on time:

  • Why you were unable to return the equipment on time
     
  • When the equipment will be returned

If the correct information is not provided, you will be asked to complete the form again.

If you have been blocked or banned because you did not return equipment on time, we require the following: 

  • An explanation of why the equipment was not returned on time. 
  • An explanation of why the form was not filled out before equipment was overdue. 

If the correct information is not provided, you will be asked to complete the form again. 

If you are not on the approved checkout list, please contact your professor and ask them to complete the request form on the faculty resources page. 

 

This should include a description of the equipment, model number, and tag numbers.
